New Communities Initiatives is now accepting applications for the Comprehensive Case Management Program and the Youth Development and Community Wellness Programs.

FY2019 New Communities Initiative Comprehensive Case Management Program

NCI is seeking applicants to provide comprehensive case management to residents of the Lincoln Heights/Richardson Dwellings community. These case management services will be offered to heads of household and their family within the same household, guided by a strengths-based case plan developed jointly by the case manager and consumer(s). Office of the Deputy Mayor for Planning and Economic Development (DMPED) will award an individual grant up to $262,400. For more information about this grant, click here

FY2019 New Communities Initiative Youth Development and Community Wellness Programs

NCI is seeking organizations to provide community health and wellness and youth development programs that focus on healthy living, and making smart choices related to health and wellness. The program should target seniors (55 years and older) as well as youth (up to age 24). While organizations serving all NCI neighborhoods may apply and will be considered, organizations proposing to serve Lincoln Heights/Richardson Dwellings will receive preference.
